General Registration Information
ONLINE REGISTRATION CLOSES
Wednesday, APRIL 9
- Take advantage of discounted registration fees by registering by April 2, 2008 (11:59 PM EST).

- Registrations canceled 30 or more days prior to the start of the Annual Meeting (by April 3rd) are fully refundable, minus a $50 administrative fee, upon written notice of cancellation. Registrations canceled 14-29 days prior to the start of the Annual Meeting (April 4-19) are refundable at 50%, upon written notice of cancellation. Registrations canceled 13 or fewer days prior to the start of the Annual Meeting (after April 19) are non-refundable.
- Please send your written cancellation and refund request to Hadijah Kagolo at hadijah@aapm.org or fax your request to 301-209-3343 (Attn: Hadijah).
- If you have registered at the discounted rate and are paying by check, your payment must be received no later than April 2 (11:59 PM EST). If we do not receive your payment for the discounted registration fees by April 2, your registration type will be changed to reflect the non-discounted registration fee.
- Already registered and need a receipt, please email Hadijah Kagolo at hadijah@aapm.org or fax your request to 301-209-3343 (Attn: Hadijah).

| MOC/SAMs SELF ASSESSMENT MODULES |

| Completion of two Self Assessment Modules (SAMs) per year is a requirement for the Maintenance of Certification (MOC) process as defined by the American Board of Radiology (ABR). ACMP program organizers have identified sessions to be offered with audience response technology at the annual meeting in 2008. |
